Pre Payment *

Payment for adjustments *

• I understand that adjustments made on the day will need to be paid on the day by cash, card or BACS. *

Cancellation Charges *

• I understand that cancellation charges apply: within 24 hours of appointment – full cost of consultation fee retained. Within 48 hours of appointment – 50% of the consultation fee will be retained. If I have to cancel or postpone, the booking fee will be retained for the new agreed date or will be refunded if no future date is agreed. *

GDPR Agreement *
